My fee is $150/session. I accept checks, Visa, Mastercard, and American Express.

I accept MHN, United HealthCare, and Optum insurance. I can also provide you with a bill to submit for reimbursement.

Your fee is due at the time of each session. A 24-hour notice is required to cancel an appointment without incurring a charge. You may miss one time without being charged. Thereafter, you are responsible for paying your set fee for all missed appointments, regardless of the reason for your absence, if you do not cancel with 24-hour notice. This does not apply to scheduled absences for vacations or personal business.
